佩德拉萨是西班牙的一个偏远小镇，被莫名其妙的恶魔事件所困扰，大多数居民都失去了理智，被关在精神病院里。陷入昏迷的Elena（Megan Montaner饰）躺在马德里医院的病床上，因悔恨而崩溃的Paco（Miguel ángel Silvestre饰）试图照顾她。随着恐怖的加剧，主人公们必须面对一个神秘的新敌人（Paul Giamatti饰）——一个扭曲到连恶魔都害怕的人。
